Your 7th grade child should have one additional vaccine by January 2016. An appropriate medical or religious exemption is also acceptable. Please check your child’s records to be sure that he/she has the following:

Hepatitis A Vaccine – 2 doses
Students now in grades 5-10 will be required to have hepatitis A vaccine by school year 2015-2016. Hepatitis A is a virus that can cause liver damage. Hepatitis A vaccine is a two dose series. The second dose should be given 6-12 months after the first dose. An appropriate medical or religious exemption is also acceptable.
If your child does not have the two doses of the Hepatitis A or the Tdap vaccination, contact your child’s doctor, the Crook County Health Department (541-447-5165) or the School Based Health Center located at Crooked River Elementary (541-323-4272) to make an appointment.
It is important to get an early start on updating your child’s immunizations. Please give all updated immunization information to your school as soon as your child receives the vaccine.
